  • Medical

    Your annual physical with a primary care doctor. $0 if you're covered by a Community Health Care medical plan.

  • Dental

    A cleaning and exam to keep your teeth and gums healthy. $0 if you're covered by a Community Health Care dental plan.

  • Vision

    An eye exam to check your vision and screen for conditions like glaucoma. This is part of your medical coverage. $20 copay if you're covered by a Community Health Care medical plan.
