Preventive care is what exactly?
Preventive care is the check-up you get every year even when you're feeling fine. It's a status check of how you feel and how you're doing physically and mentally, outwardly and inwardly.

Medical
Your annual physical with a primary care doctor. $0 if you're covered by a Community Health Care medical plan.
- A review of your health history
- Blood pressure, height, weight and BMI check
- Blood work (cholesterol, blood sugar and more)
- Screenings based on your age, gender and family history
- Hear from Kaiser on:
- Age-appropriate vaccines. Learn more
Important: If you bring up health concerns during your visit (for example, knee pain or a sore throat), the provider might code it at least partially as a diagnostic visit, which means you could be charged a fee.

Dental
A cleaning and exam to keep your teeth and gums healthy. $0 if you're covered by a Community Health Care dental plan.
- Cleaning and polishing
- X-rays (typically once a year)
- Check for cavities, gum disease and oral health issues

Vision
An eye exam to check your vision and screen for conditions like glaucoma. This is part of your medical coverage. $20 copay if you're covered by a Community Health Care medical plan.

If you don't have a primary care doctor, you're not alone. It's easier to find one than you might think, and it's so important. (You have a preferred car repair shop, right? Because it helps to have someone who knows your vehicle!)

Filter by "Family Medicine," "Obstetrics and Gynecology," "Pediatrics" or "Optometry," depending on the type of provider you're looking for.
Base Plan HMO is the "Core HMO Network."
Buy-Up Plan PPO is the "Access PPO Network."

- Look for a doctor close to home or work so scheduling is easier.
- Filter online by gender or languages spoken, or call Kaiser at 888-844-4607 for help finding a doctor of a specific ethnicity/race.

Get paid to go for a check-up!
You can get $50 every year for going to a wellness or preventive care appointment, if you're enrolled in Critical Illness, Accident or Hospital Indemnity coverage. The same is true for your covered spouse and/or children.
Eligible appointments and screenings include the following:
- Annual physical exam
- Mammogram
- Colonoscopy
- Blood test for triglycerides
- Pap test
- Serum test for HDL/LDL
- Fasting blood glucose test
- Breast ultrasound or breast MRI
- Child's sports physical
